The exchanges work like this, Insurance companies try to predict costs ahead of time to set rates, people sign up, there is bunch of pool money and the state refunds the poor people. So as you can see, not all of the money goes to the insurance companies, so the insurance companies have to factor that in as well. So who gets soaked. The middle class.
