I will guess Kent Tekulve.  The Pirates won the 1979 World Series and he was the closer.  Taking a shot that he closed out that World Series.

But he pitched for the Reds in 1989 and the Reds did not go to the World Series then...

But it says that they pitched in the last game, not necessarily the last pitcher...

As for the answer, forgot about him...  Great question from @patweisers44...

I immediately looked it up also.  And even with knowledge of the '79 team I couldn't get it. Nearly impossible.

The pitcher had to have pitched for the Pirates and Phillies.  I only know that because I live in the Philadelphia market and something like this has been said before.  The only other pitcher on that 1979 Pirates team is Don Robinson.  I didn't think he pitched that late into the '80's though.  Maybe he did... so I'll go with Don Robinson as my guess.

Robinson didn't pitch for the Phillies until 1992...

Only how I figured it out.  I knew the pitcher pitched for both the Pirates and Phillies.  I knew that because it's been talked about in Philly in the past (I live in the Philly market).  I knew that both Tekulvy and Robinson pitched for both the Pirates and Phillies... I just didn't think that Robinson pitched that many years.
