Article doesn't say but you have to think it's because the surgery and rehab is going to be extraordinarily expensive. Insurance companies lose money whenever they have to pay out. In cases like this, they're combing through the fine print for the clauses they purposely installed and buried deep in the long boring legal forms you didn't read but that they made you sign when you joined.

The reason they're not paying? It's right here in this paragraph on page 47...

As a follow-up to the prior Luigi Mangione tweet:  settle yourselves!  He still faces murder charges in state court (25-life sentence) and still faces a relatively open and shut prosecution in federal court where (for now anyway) the death penalty is on the table. His political soapbox that would have played in the state terrorism case will not be allowed at his federal trial (if he doesn't plead to life in prison to stay alive)
