For a hit where the victim is wholly unaware of the shooter's presence prior to the first shot I believe in the reverse order, first one to thed head then two to the heart. There was no reason in this scenario for the shooter to not place the barrel of his weapon against the victim's skull as he pulled the trigger.

Pretty sure i read that in a Miss Manners column in the Washington Post. She knows the proper protocol for any occassion. For instance her column on the rules for when one can leave an upper decker was very informative and spot on.
