The beginning part is talking about cancellations and no-shows of appointments, who are likely to have insurance, which would likely mean they are here legally. Interestingly the rate of increase in no shows and cancellations is significantly higher than the rate of decrease of ER visits (which are more likely to be illegals than people with appointments).
