There have been over 200 - 3 pitch innings.   not sure how many, if any had a baserunner. 

No pitcher has done it thrice, but quite a few have done it twice.

It seems it shouldn't be terribly infrequent. Hitters go after the first pitch frequently.... do it unsuccessfully three times in a row seems no big deal .. but there's a massive gap in both leagues between 1919 and 1991 with just a small handful the entire time.
