Its definitely the meth thing, and its not said in jest. Saying somebody is a "tweaker" basically means that you should stay the hell away from them.

No, it is wrong to use it on potheads. It refers to a specific set of behaviors only seen in people strung out on meth too long. Its very different than what you see from a long-time pothead or even a crack-head.

Usually to me it means stoned or drunk. But mostly stoned.

You'll also notice useless crap piled up that while tweekin, seemed like an extremely valuable item at the time. An example of this might be an old useless bike rim or a fockin pogo stick.

I've pretty much heard it used in this context:

 

My buddy: I ran into Steve the other day.

Me: Steve? I didn't know he was back in town.

My buddy: Yeah, he just got out of prison again.

The key is that a tweaker will never get any better. Meth permanently f*cks your brain...there is just no coming back from it. So thats why it is exclusive from drugs like pot or coke.
