I don’t think there’s much difference between “severe disease” and hospitalization.  The vaccines are ~80-90%+ effective at preventing hospitalization according to most data.  I think the “need” for the third jab is still up for debate.

Because it says that the trial lasted ~9 months, and once the vaccine was approved it was available to anyone including those in the placebo group (and the comments included first hand accounts of those in the placebo group who later got the real vaccine). Similar to the covid vaccine trials.
